Often referred to by officers as the "blue form," because it always came in a packet provided by the State in the color blue, this form is given to folks who are involved in motor vehicle accidents who either 1) have insurance but cannot provide the information at the time of the accident or 2) they do not have insurance and have damaged the property of another. This form is to be filled out by the citizen and mailed to the State at the address on the form.

Use this form when you've been in an accident, but the police did not attend. The State of NH advises "In the State of New Hampshire, any motor vehicle accident causing death, personal injury, or combined vehicle/property damage in excess of $1,000.00 must be reported in writing to the NH DMV within 15 days."
 
If you need to request a continuance from the court or need a blank motion for any other reason, this is the form you want. 
 
You may also find a change of address form, which the court requires if you move while you have an open case. The court will require you to give the police department a copy after it's been provided to the court.

For when the State/Town Clerk requests that an officer performs a "VIN check" on your boat. An officer is glad to come to you to provide this service.
 
This is a State form. Please fill it out (it is a type-able form), print, sign, and date. This can be dropped off in person, emailed to chesterpd@chesternhpolice.org or snail-mailed to 84 Chester St, Chester, NH 03036. There is a $10 fee, which can be paid after the permit is approved. We can accept only cash, checks, and money-orders at this time.
 
Use this form to request an accident report, incident report, or an adjudicated (closed) arrest report. Provide this form to the Chester Police front desk for processing. The fee scale for reports is as follows: first 5 pages are free and each additional page is $0.50; accident reports are a flat $15 fee.
 
Use this form for open cases that are still in the court process. This form should be sent directly to the Prosecutor's office, which is out of the Derry Police Department at 1 Municipal Drive in Derry, NH 03038 or signed and emailed to dpdprosecution@derrynh.org
 
If you prefer to hand-write a statement, feel free to print this form and send it to us. Otherwise, you can write your statement in the body of an email and send it to chesterpd@chesternhpolice.org  Just be sure to include your date of birth and phone number.
 
As a service to the community, the Chester Police Department accepts vacant house notifications.  When this form is completed and turned in at the station, officers will be notified your residence is vacant and be able to contact you in case of an emergency in your absence.
